This is the shared route for prayer notes for health or repose. Names are stored in the private Supabase office workflow rather than being sent as an exposed email thread.

Name limit

Up to 15 names per submission so review and printing stay straightforward.

Storage

The data goes only to the private office queue used by clergy and designated church workers.

Languages

The form is available in Ukrainian, Czech, and English without a separate account.

One form for both note types

Visitors do not need to hunt for separate routes. They can choose health or repose within one calm form.

Private workflow boundary

Prayer notes stay out of the public CMS and go straight into the narrow office workflow for review and print preparation.

Clear confirmation

After submission, the visitor gets an immediate success or error state without exposing unnecessary details on screen.
